Need support completing your application for a birth certificate, status card or Ontario Photo Card? Stop by our walk-in clinic.

What to bring with you:

• Personal information: full legal name, date of birth, place of birth, name of hospital

• Parent information: full names (and mother’s maiden name), dates and places of birth of your biological parents

• Guarantor name: someone in an approved profession (teacher, chief, social worker, etc) who has known you for at least 2 years
